Acne is the most common skin disorder in the United States. Contrary to popular belief, it is not a condition that affects only young adults but affects grown adults as well. Hormonal changes, diet, stress and environmental influences are all factors that can cause acne to be a frequent problem for any age. If that is not frustrating enough, the red, clogged pores can turn into a long-lasting reminder in the form of acne scars.
Acne can be caused by a multitude of different factors including:
Genetics
Hormonal changes
Excess oil production
Blocked pores
Medications
Humidity or pollution
Inflammatory diet
Inflammation from cutibacterium acnes

How Acne Happens:
There are certain key elements that play a role in developing large painful pimples. First, in predisposed patients, the skin cells lining the pores stick together and form a plug on the skin surface, known as a white head or a black head. Secondly, overactive sebaceous oil glands produce an excessive amount of oil which causes proliferation of the bacteria Cutibacterium acnes (previously known as Propionibacterium acnes) within the blocked pore. This bacterial overgrowth leads to inflammation which appears as red bumps or pustules. In some cases, there is a marked inflammatory response and the follicle wall ruptures which may lead to scarring.
Certain factors can trigger or worsen acne, including genetic predisposition, diet, menstruation, hormones and stress.
Outbreaks are not only visible and unattractive, they are painful and embarrassing. Waking up with pimples or acne cysts can affect confidence and self-esteem, no matter the age.
Treatment
Different treatments target different stages responsible for acne development. Treatments are tailored to address each of the stages and often consist of a combination of more than one treatment option. The goal is to treat the current outbreak and prevent future ones from occurring.
Some of the different treatment options include:
Oral antibiotics
Topical antibiotics
Chemical peels (glycolic, salicylic, TCA)
Retinoids
Laser
Light therapy
Chemical peels are a great way to control current acne breakouts. More specifically, the Jessner peel was designed to deconstruct the top-most layer of the skin, dry out active acne, dislodge blackheads, unclog pores, which also leads to the removal of actinic keratosis (sun spots). Photodynamic therapy shrinks oil glands, decreases inflammation, kills Propionibacterium acnes (bacteria contributing to acne), and improves discoloration from old acne lesions. IPL treatment and salicylic acid peels can lessen the appearance of pigmentation following acne.
Successful treatment of acne consists of a multi-step approach including short-term treatment (controlling acute inflammation) and long-term prevention. The treatment plan will depend on each case and will be custom-tailored to address the underlying cause of your outbreaks. It is important to identify the underlying cause of your acne in order to develop a comprehensive treatment plan and definitive treatment.
Acne scarring can be addressed once active acne is under control. Combination treatment is usually needed in addition to early, appropriately aggressive intervention.
